Reporter
The individual submitting the COI disclosure, typically includes faculty members, researchers, research staff, and graduate students.
The Disclosure Form may include additional sections or questions based on your responses. Be sure to answer all primary and follow-up questions. If you need to exit and return later, click SAVE to keep your progress.
If your initial disclosure has been completed and dispositioned, please follow the instructions to create a new version.
If your disclosure has been Returned for revisions, please follow the instructions to submit a returned disclosure.

After your initial submission, you will receive email reminders from Kuali Notifications before your disclosure expires, based on the submission date.
University employees entering into a Consulting or related agreement with an external entity must provide a signed copy of the internal Memorandum of Understanding (MOU), to the external entity for which the employee is Consulting. (The same signed copy suffices for all external entities).
Unit Head/Chair/Director/Dean
Responsible for completing a thoughtful review of the disclosure, using situational awareness.
Reviewers include Department Chairs, Unit Heads, College Deans, and Center Directors. Before their review is complete, reviewers will confirm the following statement: “I certify I have reviewed and approved the information provided in this disclosure form. To the best of my knowledge, a full disclosure of reportable conflicts of interest and outside activities has been submitted. I understand that further review may be conducted by other Senior Administrators, Office of Research Compliance, the Conflict of Interest Committee, and a Management Plan may be implemented.”
Based on their review of the disclosure, reviewers assess whether a conflict of interest exists and determine if a management plan is needed to address it. These assessments are crucial for supporting the Disclosure Office and the Conflict of Interest Committee in completing their evaluation and making final dispositions. Reviewers confirm they have read, commented on, and believe all pertinent information is disclosed to the best of their knowledge. They are responsible for reviewing and signing off on the disclosure before its submission to the COI Committee, selecting an option to indicate whether they believe a conflict exists and if a management plan is necessary.
The Disclosure Office has created guidelines for reviewers to follow when reviewing and assessing information provided in disclosures.
COI Committee
Appointed members of UMD community responsible for performing an advanced review of COI disclosures.
Committees: COI/OCI/ICOI
Committees assist in the development of management plans/mitigation plans, review developed plans; ensure compliance with policies, and make recommendations to the President.
University President is the “approving” authority in the COI process.
Agency sponsor is the "approving" authority in the OCI process.

Individuals who may need access to or view the disclosure and/or management plan include the Director of Administrative Services, College Business Officers, ORA, and COI Committee Members.
